Regrettably a privacy breach has recently occurred involving two survivors. This isolated, yet serious incident was a result of human error and the staff involved are understandably devastated.

A journalist was made aware of the situation resulting in potential media coverage. The Royal Commission has responded to the journalist.

We have been assured that the information that was shared has been destroyed.

We are deeply sorry that this happened. We are treating this matter very seriously - it is under full investigation.

The Royal Commission has also notified the Office of the Privacy Commissioner and will fully cooperate with them as needed.

Both survivors have been contacted: the person whose information was breached, and the person who the information was sent to in error. We have given our sincere apologies and offered appropriate support. We will continue to work with the individuals involved.

Our procedures are robust, however in light of this occurrence, they are being reviewed so that we can confidently assure survivors that the information they share with us will be kept safe going forward. We assure you our commitment to ensuring survivor confidentiality and wellbeing remains steadfast.
